83d290c56f SPDX: Convert all of our single license tags to Linux Kernel style
When U-Boot started using SPDX tags we were among the early adopters and
there weren't a lot of other examples to borrow from. So we picked the
area of the file that usually had a full license text and replaced it
with an appropriate SPDX-License-Identifier: entry. Since then, the
Linux Kernel has adopted SPDX tags and they place it as the very first
line in a file (except where shebangs are used, then it's second line)
and with slightly different comment styles than us.

In part due to community overlap, in part due to better tag visibility
and in part for other minor reasons, switch over to that style.

This commit changes all instances where we have a single declared
license in the tag as both the before and after are identical in tag
contents. There's also a few places where I found we did not have a tag
and have introduced one.

8627733941 ti816x: Rework DDR initialization sequence
The ti816x/am389x SoC is the first generation in what U-Boot calls the
"am33xx" family. In the first generation of this family the DDR
initialization sequence is quite different from all of the subsequent
generations. Whereas with ti814x (second generation) we can easily work
the minor differenced between that and am33xx (third generation), our
attempts to do this for ti816x weren't sufficient. Rather than add a
large amount of #ifdef logic to make this different sequence work we add
a new file, ti816x_emif4.c to handle the various required undocumented
register writes and sequence and leverage what we can from
arch/arm/mach-omap2/am33xx/ddr.c still. As DDR2 has similar problems
today but I am unable to test it, we drop the DDR2 defines from the code
rather than imply that it works by leaving it. We also remove a bunch
of other untested code about changing the speed the DDR runs at.

c4f80f5003 am335x_evm / gumstix pepper: Correct DDR settings
As noted by clang, we have been shifting certain values out of 32bit
range when setting some DDR registers. Upon further inspection these
had been touching reserved fields (and having no impact). These came in
from historical bring-up code and can be discarded. Similarly, we had
been declaring some fields as 0 when they will be initialized that way.
Tested on Beaglebone White.

58c86c7d1d am33xx: fix the ddr_cmdtctrl structure
Fix the wrong mapping between the DDR I/O control registers on AM33XX
SoCs and the software representation in the SPL code.
The most recent public TRM defines the following DDR I/O control registers
offsets:
 * ddr_cmd0_ioctrl : offset 0x44E11404
 * ddr_cmd1_ioctrl : offset 0x44E11408
 * ddr_cmd2_ioctrl : offset 0x44E1140C
 * ddr_data0_ioctrl: offset 0x44E11440
 * ddr_data1_ioctrl: offset 0x44E11444

While the struct ddr_cmdtctrl has also some reserved bits in the beginning.
The struct is mapped to the address 0x44E11404. As a result "cm0ioctl" points
to the ddr_cmd1_ioctrl register, "cm1ioctl" to the ddr_cmd2_ioctrl and etc.
Registers ddr_cmd0_ioctrl and ddr_data0_ioctrl are never configured because
of this mapping mismatch.

The ddr_regs struct was incorrectly offset after the dt0wiratio0 entry.
Correct this by documenting a missing register that will be used at some
