The code was processed with unifdef utility to omit portions not
relevant to A38x and DDR3. This removes usage of many macros, including
A70X0, A80X0 and A3900. It seems that the unifdef utility did not remove
the macros from #else comment.

Bump version of a38x DDR3 trianing to version 14.0.0 to reflect the
version in the mv-ddr-devel branch of upstream repository

In commit 6285efb ("mv_ddr: add support for twin-die combined memory
device") was added support for twin-die combined memory device and
default value for explicitly uninitialized structure members is zero, s
also twin_die_combined is initialized to zero. Which means COMBINED
value.
As prior this commit there was no support for twin-die combined memory
device, default value for twin_die_combined should be NOT_COMBINED. This
change change order of enum mv_ddr_twin_die to ensure that NOT_COMBINED
has value zero.

The funtion returnd cs size in byte instead of MB, that cause
calculation error since the caller was expected to get u32 and when he
got above 4G it refers it as 0.
The fix was to get the cs memory size from function as in MB and then
multiply it by 1MB.

The ODT enable heuristic based on active chip-selects is not always
correct. Some board might use two chip-selects, but have only one ODT
line connected. Allow board specific mv_ddr_topology_map to directly set
the ODT configuration register value.

In the Armada 385 functional spec (MV-S109094-00 Rev. C) the read sample
delay fields are 5 bits wide. Use the correct bitmask of 0x1f when

DDR clock out might be unrelated to the number of active chip-select.
For example, the board might have two DDR components, but only one
chip-select. The clk_enable mask allows the board to enable DDR clocks
regardless of active chip-selects.

Based on the JEDEC standard JESD79-3F. The tRAS timings should include
the highest speed bins at a given frequency. This is similar to commit
683c67b ("mv_ddr: ddr3: fix tfaw timimg parameter") where the wrong
comparison was used in the initial implementation.

No in-tree code defines SUPPORT_STATIC_DUNIT_CONFIG or
STATIC_ALGO_SUPPORT. Remove ddr3_a38x_mc_static.h and use unifdef to
remove unused sections in the rest of the ddr/marvell/a38x code.

PEX_CFG_DIRECT_ACCESS was defined in ddr3_hws_hw_training_def.h despite
only being used in the serdes code. Move this definition to ctrl_pex.h
where all the other PEX defines are. Also remove the duplicate
definition of PEX_DEVICE_AND_VENDOR_ID which is already defined in
ctrl_pex.h.

When U-Boot started using SPDX tags we were among the early adopters and
there weren't a lot of other examples to borrow from. So we picked the
area of the file that usually had a full license text and replaced it
with an appropriate SPDX-License-Identifier: entry. Since then, the
Linux Kernel has adopted SPDX tags and they place it as the very first
line in a file (except where shebangs are used, then it's second line)
and with slightly different comment styles than us.
In part due to community overlap, in part due to better tag visibility
and in part for other minor reasons, switch over to that style.
This commit changes all instances where we have a single declared
license in the tag as both the before and after are identical in tag
contents. There's also a few places where I found we did not have a tag
and have introduced one.

The RD_SAMPLE_DELAY field is 5 bits so it needs to be masked with 0x1f
instead of 0xf. Rather than checking the read sample delay for all DDR
chip selects use the values for the chip selects that are actually
configured. Finally continue searching for the max_phase value even if the
current read_sample is the same as the max_read_sample.

When using only a single DDR chip select only assert M_ODT[0] on write.
Do not assert it on read and do not assert M_ODT[1] at all. Also set
tODT_OFF_WR to 0x9 which contradicts the recommendation from the
functional spec but is what Marvell's binary training blob does and
seems to give better results when ODT is active during writes.
