b335fe6810 usb.h: Always declare usb function prototypes
There is no harm in declaring the function prototypes even if nothing
implements them, and when CONFIG_DM_USB=y the various usb functions are
available regardless of any controller drivers being enabled.

This fixes compile warnings due to missing prototypes on ARCHs where
the ARCH Kconfig always enables CONFIG_DM_USB and various usb drivers.

One could argue that in the case of no controllers CONFIG_DM_USB should not
be set, but this problem is typically seen during bringup of boards which
do actually have usb controllers.

b6de4d1093 dm: usb: Add support for companion controllers
USB companion controllers must be scanned after the main controller has
been scanned, so that any devices which the main controller which to hand
over to the companion have actually been handed over before we scan the
companion.

As there are no guarantees that this will magically happen in the right
order, split the scanning of the buses in 2 phases, first main controllers,
and then companion controllers.

8a5f0665da dm: usb: Add support for interrupt queues to the dm usb code
Interrupt endpoints typically are polled for a long time by the usb
controller before they return anything, so calls to submit_int_msg() can
take a long time to complete this.

To avoid this the u-boot code has the an interrupt queue mechanism / API,
add support for this to the driver-model usb code and implement it for the
dm ehci code.

7f1a07538f dm: usb: Copy over usb_device values from usb_scan_device() to final usb_device
Currently we copy over a number of usb_device values stored in the on stack
struct usb_device probed in usb_scan_device() to the final driver-model managed
struct usb_device in usb_child_pre_probe() through usb_device_platdata, and
then call usb_select_config() to fill in the rest.

There are 3 problems with this approach:

1) It does not fill in enough fields before calling usb_select_config(),
specifically it does not fill in ep0's maxpacketsize causing a div by zero
exception in the ehci driver.

2) It unnecessarily redoes a number of usb requests making usb probing slower

3) Calling usb_select_config() a second time fails on some usb-1 devices
plugged into usb-2 hubs, causing u-boot to not recognize these devices.

This commit fixes these issues by removing (*) the usb_select_config() call
from usb_child_pre_probe(), and instead of copying over things field by field
through usb_device_platdata, store a pointer to the in stack usb_device
(which is still valid when usb_child_pre_probe() gets called) and copy
over the entire struct.

*) Except for devices which are explictly instantiated through device-tree
rather then discovered through usb_scan_device() such as emulated usb devices
in the sandbox.

0566e2403d dm: usb: Allow USB drivers to be declared and auto-probed
USB devices in U-Boot are currently probed only after all devices have
been enumerated. Each type of device is probed by custom code, e.g.:

- USB storage
- Keyboard
- Ethernet

With driver model this approach doesn't work very well. We could build
a picture of the bus and then go back and add the devices later, but
this means that the data structures are incomplete for quite a while.
It also does not follow the model of being able to bind a device when we
discover it.

We would prefer to have devices automatically be bound as the device is
enumerated. This allows us to attach drivers to particular USB classes
or product/vendor IDs. This is the method used by Linux.

Add the required #defines from Linux, a way of declaring a USB driver and
the logic to locate the correct driver given the USB device's descriptors.

90cdc1039d musb-new: Fix reset sequence when in host mode
This commit fixes a number of issues with the reset sequence of musb-new
in host mode:

1) Our usb device probe relies on a second device reset being done after the
first descriptors read. Factor the musb reset code into a usb_reset_root_port
function (and add this as an empty define for other controllers), and call
this when a device has no parent.

2) Just like with normal usb controllers there needs to be a delay after
reset, for normal usb controllers, this is handled in hub_port_reset, add a
delay to usb_reset_root_port.

3) Sync the musb reset sequence with the upstream kernel, clear all bits of
power except bits 4-7, and increase the time reset is asserted to 50 ms.

With these fixes an usb keyboard I have now always enumerates properly, where
as earlier it would only enumerare properly once every 5 tries.

6e9e06260d usb: dwc2: Add driver for Synopsis DWC2 USB IP block
This is the USB host controller used on the Altera SoCFPGA and Raspbery Pi.

This code has three checkpatch warnings, but to make sure it stays at least
readable and clear, these are not fixed. These bugs are in the USB request
handling combinatorial logic, so any abstracting of those is out of question.

Tested on DENX MCV (Altera SoCFPGA 5CSFXC6C6U23C8N) and RPi B+ (BCM2835).

75504e9592 usb: dfu: fix boards wo USB cable detection
Former usb_cable_connected() patch broke compilation of boards which do
not support this feature.

I've renamed usb_cable_connected() to g_dnl_usb_cable_connected() and added
its default implementation to gadget downloader driver code. There's
only one driver of this kind and it's unlikely there'll be another, so
there's no point in keeping it in /common.

Previously this function was declared in usb.h. I've moved it, since
it's more appropriate to keep it in g_dnl.h - usb.h seems to be intended
for USB host implementation.

Existing code, confronted with default -EOPNOTSUPP return value,
continues as if the cable was connected.

CONFIG_USB_CABLE_CHECK was removed.

351e9b2069 usb: ums: add ums exit feature by ctrl+c or by detach usb cable
This patch allows exiting from UMS mode to u-boot prompt
by detaching usb cable or by pressing ctrl+c.

Add new config: CONFIG_USB_CABLE_CHECK. If defined then board
file should provide function: usb_cable_connected() (include/usb.h)
that return 1 if cable is connected and 0 otherwise.

5853e1335c USB: xHCI: Add stack support for xHCI
This adds stack layer for eXtensible Host Controller Interface
which facilitates use of USB 3.0 in host mode.

Adapting xHCI host controller driver in linux-kernel
by Sarah Sharp to needs in u-boot.

Initial porting from Linux kernel version 3.4, with following
top commit history of drivers/usb/host/xhci* :
cf84055 xHCI: Cleanup isoc transfer ring when TD length mismatch found

This adds the basic xHCI host controller driver with bare minimum
features:
- Control/Bulk transfer support has been added with required
  infrastructure for necessary xHC data structures.
- Stream protocol hasn't been supported yet.
- No support for quirky devices has been added.

833a53c627 musb-new: am35x backend driver
Backend driver for MUSB OTG controllers found on TI AM35x.

It seems that on AM35X interrupt status registers can be updated
_before_ core registers. As we don't use true interrupts in U-Boot
and poll interrupt status registers instead this can result in
interrupt handler being called with non-updated core registers.
This confuses the code and result in hanged transfers.
Add a small delay in am35x_interrupt as a workaround.

c60795f41d usb: use linux/usb/ch9.h instead of usbdescriptors.h
Linux usb/ch9.h seems to have all the same information (and more)
as usbdescriptors.h so use the former instead of the later one.

As a consequense of this change USB_SPEED_* values don't correspond
directly to EHCI speed encoding anymore, I've added necessary
recoding in EHCI driver. Also there is no point to put speed into
pipe anymore so it's removed and a bunch of host drivers fixed to
look at usb_device->speed instead.

Old usbdescriptors.h included is not removed as it seems to be
used by old USB device code.

This makes usb.h and usbdevice.h incompatible. Fortunately the
only place that tries to include both are the old MUSB code and
it needs usb.h only for USB_DMA_MINALIGN used in aligned attribute
on musb_regs structure but this attribute seems to be unneeded
(old MUSB code doesn't support any DMA at all).

71c5de4f4a ehci-hcd.c, musb_core, usb.h: Add USB_DMA_MINALIGN define for cache alignment
The USB spec says that 32 bytes is the minimum required alignment.
However on some platforms we have a larger minimum requirement for cache
coherency.  In those cases, use that value rather than the USB spec
minimum.  We add a cpp check to <usb.h> to define USB_DMA_MINALIGN and
make use of it in ehci-hcd.c and musb_core.h.  We cannot use MAX() here
as we are not allowed to have tests inside of align(...).

f57661394a USB: Align buffers at cacheline
This avoids cache-alignment warnings shown in console
when a usb command is entered.

Whenever X bytes of unaligned buffer is invalidated, arm core
invalidates X + Y bytes as per the cache line size and throws
these warnings.

f9636e8d38 USB: move keyboard polling into kbd driver
This moves keyboard polling logic from USB HCD drivers into USB
keyboard driver.  Remove usb_event_poll() as keyboard polling was
the only user of this API.  With this patch USB keyboard works with
EHCI controllers again.  Tested on a tegra2 seaboard.

80b350a7ab usb: increase non-bulk timeout for slow chipsets.
If you take a look at 96820a35, you'll see the original timeout was
CONFIG_SYS_HZ.  Which is 1000.  After the mentioned change, non-bulk timeout
was changed to 100.  This causes timeout failures on the dreamplug platform
when trying to initialize the usb microsd reader.

89d48367ed Add USB host ethernet adapter support
This adds support for using USB Ethernet dongles in host mode. This is just
the framework - drivers will come later. A new config option called
CONFIG_USB_HOST_ETHER can be defined in board config files to switch this
on.

The was originally written by NVIDIA and was cleaned up for release by the
Chromium authors.

96820a3587 Fix EHCI usb submit timeout and unify with OHCI
Changed both to use a common timeout for URB submission, since they were using
different values and EHCI's was too short.

Also fixed EHCI to actually check if urb submission succeeded, rather than
silently continuing into the weeds.

d0fe1128c4 USB: fix create_pipe()
create_pipe() can give wrong result if an expression is passed as the 'endpoint'

f298e4b6dd OMAP3 Add usb device support
This change adds the usb device support for musb.

Omap3 platform support added at the same level as davinci.

The interface for usbtty to use the musb device support was added.

Verified on omap3 beagle, zoom1 and zoom2.

8f8bd565f3 USB Consolidate descriptor definitions
The header files usb.h and usbdescriptors.h have the same nameed
structure definitions for

usb_config_descriptor
usb_interface_descriptor
usb_endpoint_descriptor
usb_device_descriptor
usb_string_descriptor

These are out right duplicates in usb.h

usb_device_descriptor
usb_string_descriptor

This one has extra unused elements

usb_endpoint_descriptor

	unsigned char	bRefresh
	unsigned char	bSynchAddress;

These in usb.h have extra elements at the end of the usb 2.0
specified descriptor and are used.

usb_config_descriptor
usb_interface_descriptor

The change is to consolidate the definition of the descriptors
to usbdescriptors.h.  The dublicates in usb.h are removed.
The extra element structure will have their name shorted by
removing the '_descriptor' suffix.

So

usb_config_descriptor -> usb_config
usb_interface_descriptor -> usb_interface

For these, the common descriptor elements are accessed now
by an element 'desc'.

As an example

-	if (iface->bInterfaceClass != USB_CLASS_HUB)
+	if (iface->desc.bInterfaceClass != USB_CLASS_HUB)

This has been compile tested on MAKEALL arm, ppc and mips.
