This converts the following to Kconfig:
CONFIG_SKIP_LOWLEVEL_INIT
CONFIG_SKIP_LOWLEVEL_INIT_ONLY
In order to do this, we need to introduce SPL and TPL variants of these
options so that we can clearly disable these options only in SPL in some
cases, and both instances in other cases.

As the logic here is only used when we have a USB host controller, test
on CONFIG_USB_HOST rather than CONFIG_USB in general. This lets us move
towards using CONFIG_USB only as a menu symbol to say that we have some
form of USB, and then USB_HOST or USB_GADGET depending on the role that
USB plays within the build.

When U-Boot started using SPDX tags we were among the early adopters and
there weren't a lot of other examples to borrow from. So we picked the
area of the file that usually had a full license text and replaced it
with an appropriate SPDX-License-Identifier: entry. Since then, the
Linux Kernel has adopted SPDX tags and they place it as the very first
line in a file (except where shebangs are used, then it's second line)
and with slightly different comment styles than us.
In part due to community overlap, in part due to better tag visibility
and in part for other minor reasons, switch over to that style.
This commit changes all instances where we have a single declared
license in the tag as both the before and after are identical in tag
contents. There's also a few places where I found we did not have a tag
and have introduced one.

We have a large number of places where while we historically referenced
gd in the code we no longer do, as well as cases where the code added
that line "just in case" during development and never dropped it.

In order to add a generic MIPS debug_uart_init call right before the call to
board_early_init_f, we need to remove all calls to debug_uart_init from every
MIPS boards.
WDR4300 doesn't provide a board_debug_uart_init and configures pinmux in
board_early_init_f instead. Since I have no idead of what's the needed uart
pinmux config, I copied the whole pinmux config to a new function that is
called from board_early_init_f if CONFIG_DEBUG_UART_BOARD_INIT is not enabled.

Add support for the TPLink WDR4300 router, which is based on the
AR9344 MIPS 74Kc CPU and has 128 MiB of RAM. The USB is supported
on this system as well.
