Replace all relocate_code routines from ARM start.S files
with a single instance in file arch/arm/lib/relocate.S.
For PXA, this requires moving the dcache unlocking code
from within relocate_code into c_runtime_cpu_setup.

While other architectures have enabled these gcc / ld options for some
time on U-Boot itself, ARM has only been doing this on SPL. Enable this
on full U-Boot as well now.

Commit 3ebd1cbc introduced compiler-generated __bss_start
and __bss_end__ and commit c23561e7 rewrote all __bss_end__
as __bss_end. Their merge caused silent and harmless but
potentially bug-inducing clashes between compiler- and linker-
generated __bss_end symbols.
Make __bss_end and __bss_start compiler-only, and create
__bss_base and __bss_limit for linker-only use.

Use __image_copy_end instead of __bss_start for the end of the image to
relocate. This is the same as commit 033ca72, but applied to all ARM start.S.
This is a more appropriate symbol naming for an image copy & relocate feature,
and this also saves a useless copy of data put between __image_copy_end and
__bss_start in linker scripts (e.g. relocation information, or MMU
initialization tables used only before jumping to the relocated image).

Albert's rework of the linker scripts conflicted with Simon's making
everyone use __bss_end. We also had a minor conflict over
README.scrapyard being added to in mainline and enhanced in
u-boot-arm/master with proper formatting.

Note this is a tree-wide change affecting multiple architectures.
At present we use __bss_start, but mostly __bss_end__. This seems
inconsistent and in a number of places __bss_end is used instead.
Change to use __bss_end for the BSS end symbol throughout U-Boot. This
makes it possible to use the asm-generic/sections.h file on all
archs.

Refactor linker-generated array code so that symbols
which were previously linker-generated are now compiler-
generated. This causes relocation records of type
R_ARM_ABS32 to become R_ARM_RELATIVE, which makes
code which uses LGA able to run before relocation as
well as after.
Note: this affects more than ARM targets, as linker-
lists span possibly all target architectures, notably
PowerPC.

Turn __bss_start and __bss_end__ from linker-generated
to compiler-generated symbols, causing relocations for
these symbols to change type, from R_ARM_ABS32 to
R_ARM_RELATIVE.
This should have no functional impact, as it affects
references to __bss_start and __bss_end__ only before
relocation, and no such references are done.

Move all the C runtime setup code from every start.S
in arch/arm into arch/arm/lib/crt0.S. This covers
the code sequence from setting up the initial stack
to calling into board_init_r().
Also, rewrite the C runtime setup and make functions
board_init_*() and relocate_code() behave according to
normal C semantics (no jumping across the C stack any
more, etc).
Some SPL targets had to be touched because they use
start.S explicitly or for some reason; the relevant
maintainers and custodians are cc:ed.

The command declaration now uses the new LG-array method to generate
list of commands. Thus the __u_boot_cmd section is now superseded and
redundant and therefore can be removed. Also, remove externed symbols
associated with this section from include/command.h .

In currently, when __bss_start is equal to __bss_end__,
The bss loop will clear all the things in memory space.
But just only when __bss_end__ greater than __bss_start__,
we do the clear bss section operation.

Add -EB flag to LD to switch endianness of the linker. This should make armeb
targets buildable again. Also, make use of U-Boot's internal libgcc instead of
toolchain's one, this works around the use of libraries from GCC, which might be
little endian.

U-Boot Makefiles contain a number of tests for compiler features etc.
which so far are executed again and again. On some architectures
(especially ARM) this results in a large number of calls to gcc.
This patch makes sure to run such tests only once, thus largely
reducing the number of "execve" system calls.
Example: number of "execve" system calls for building the "P2020DS"
(Power Architecture) and "qong" (ARM) boards, measured as:
-> strace -f -e trace=execve -o /tmp/foo ./MAKEALL <board>
-> grep execve /tmp/foo | wc -l

As a result, built times are significantly reduced, typically by

Monitor protection region in FLASH did not cover .rel.dyn
and .dynsym sections, because it uses __bss_start to compute
monitor_flash_len. Use _end instead.

The include/miiphy.h header duplicates a lot of things from linux/mii.h.
So punt all the things that overlap to keep the API simple and to make
merging between U-Boot and Linux simpler.

r8 is used for global_data and should therefore be left alone!
For C code the compiler flag --fixed-r8 does the job, but in assembler
we need to be aware of that fact.

In case we are still at relocation target address before relocation we
do not need to load the registers needed for relocation. We should
instead skip the whole relocation part and jump over to clear_bss
immediately.

This patch uses r1 as scratch register for copy_loop(). Therefore we do
not longer need r7 for the storage of relocate_code()'s 'addr_moni' (the
destination address of relocation).
Therefore r7 can be used later on for other purposes.

Before this commit, weak symbols were not overridden by non-weak symbols
found in archive libraries when linking with recent versions of
binutils. As stated in the System V ABI, "the link editor does not
extract archive members to resolve undefined weak symbols".
This commit changes all Makefiles to use partial linking (ld -r) instead
of creating library archives, which forces all symbols to participate in
linking, allowing non-weak symbols to override weak symbols as intended.
This approach is also used by Linux, from which the gmake function
cmd_link_o_target (defined in config.mk and used in all Makefiles) is
inspired.
The name of each former library archive is preserved except for
extensions which change from ".a" to ".o". This commit updates
references accordingly where needed, in particular in some linker
scripts.
This commit reveals board configurations that exclude some features but
include source files that depend these disabled features in the build,
resulting in undefined symbols. Known such cases include:
- disabling CMD_NET but not CMD_NFS;
- enabling CONFIG_OF_LIBFDT but not CONFIG_QE.

For ARM systems, before ELF relocation was introduced,
CONFIG_SKIP_RELOCATE_UBOOT coul be used to prevent *COPYING* the
U-Boot image from whereever it was loaded to it's link address
(CONFIG_SYS_TEXT_BASE). The name was badly chosen, as no relocation
was performed at all, it was just a memcpy().
With ELF relocation, this does not work like that any more, and
related boards need to be fixed anyway. So don't keep this relict any
longer.
