This converts the following to Kconfig:
CONFIG_USE_PREBOOT
CONFIG_PREBOOT
Both are together in one commit, since otherwise the former causes kconfig
to define the latter, which gives duplicate symbol errors.
Includes a manual fixup for CONFIG_PREBOOT in ids8313_defconfig since the
backslash lands in the wrong place. Similarly with socfpga_vining_fpga.

In order to use CONFIG_PREBOOT with Kconfig, CONFIG_USE_PREBOOT must be
defined for each board. To prepare for conversion to Kconfig, add this.

Change the configured serial console index to reflect that it is the
first physical UART, reorder some defconfig lines and move some
environment configuration from bcmstb.h to bcm7445.h.

After creating CONS_INDEX and migrating a bunch of boards to it,
there are a bunch of defined references to CONFIG_SERIALx which
are not referenced in any C code or #ifdef, so they can now be
removed

Move the contents of prior_stage.h into bcmstb.h to prevent a build
failure when bcmstb.h is #include'ed before the asm/arch symbolic link
is present.

Add support for loading U-Boot on the Broadcom 7445 SoC. This port
assumes Broadcom's BOLT bootloader is acting as the second stage
bootloader, and U-Boot is acting as the third stage bootloader, loaded
as an ELF program by BOLT.
