When U-Boot started using SPDX tags we were among the early adopters and
there weren't a lot of other examples to borrow from. So we picked the
area of the file that usually had a full license text and replaced it
with an appropriate SPDX-License-Identifier: entry. Since then, the
Linux Kernel has adopted SPDX tags and they place it as the very first
line in a file (except where shebangs are used, then it's second line)
and with slightly different comment styles than us.
In part due to community overlap, in part due to better tag visibility
and in part for other minor reasons, switch over to that style.
This commit changes all instances where we have a single declared
license in the tag as both the before and after are identical in tag
contents. There's also a few places where I found we did not have a tag
and have introduced one.

This correctly enables the USB PHY clocks, by enabling CM_ALWON_USBPHY_CLKCTRL
and correctly setting CM_L3INIT_USBPHY_CLKCTRL's value.

OMAP4470 reference design uses TWL6032 PMIC
with a following connection scheme:
VDD_CORE = TWL6032 SMPS2
VDD_MPU = TWL6032 SMPS1
VDD_IVA = TWL6032 SMPS5

Update PLL values.
SYS_CLKSEL value for 20MHz is changed to 2. In other platforms
SYS_CLKSEL value 2 represents reserved. But in sys_clk array
ind 1 is used for 13Mhz. Since other platforms are not using
13Mhz, reusing index 1 for 20MHz.

The sys_clk on the dra evm board is 20MHZ.
Changing the configuration for the same.
And also moving V_SCLK, V_OSCK defines to
arch/clock.h for OMAP4+ boards.
