At present panic() is in the vsprintf.h header file. That does not seem
like an obvious choice for hang(), even though it relates to panic(). So
let's put hang() in its own header.

Move this function header to serial.h since this function is clearly
related to serial. The function itself stays in console.c since we don't
have a single serial file. DM and non-DM each has a separate file so we
would have to either create a new common serial file, or repeat the
function in both serial.c and serial-uclass.c, neither of which seem
worthwhile.

When U-Boot started using SPDX tags we were among the early adopters and
there weren't a lot of other examples to borrow from. So we picked the
area of the file that usually had a full license text and replaced it
with an appropriate SPDX-License-Identifier: entry. Since then, the
Linux Kernel has adopted SPDX tags and they place it as the very first
line in a file (except where shebangs are used, then it's second line)
and with slightly different comment styles than us.
In part due to community overlap, in part due to better tag visibility
and in part for other minor reasons, switch over to that style.
This commit changes all instances where we have a single declared
license in the tag as both the before and after are identical in tag
contents. There's also a few places where I found we did not have a tag
and have introduced one.

For id = 15 an out of bound access occurs in udc_setup_ep().
Increase the size of epinfo[] from 30 to 32 to encompass
ids 0..15.
The problem was highlighted by cppcheck.

The endpoint rx count register value will be zero if it is read before
receive packet ready bit (PERI_RXCSR:RXPKTRDY) is set.
Check for the receive packet ready bit (PERI_RXCSR:RXPKTRDY) before
reading endpoint rx count register. Proceed with rx count read and
FIFO read only if RXPKTRDY bit is set.

This change adds the usb device support for musb.
Omap3 platform support added at the same level as davinci.
The interface for usbtty to use the musb device support was added.
Verified on omap3 beagle, zoom1 and zoom2.
