commit 3d2fc79714 ("cmd: blk: Allow generic read/write operations to work in sandbox")
used the hard-coded block size (512) for accessing the sandbox host
device. Now that we have added support for non-512 block size for both
Sandbox host device and blkmap driver, let's stop using the hard-coded
block size.

Ensure that the memory destination/source addresses of block
read/write operations are mapped in before access. Currently, this is
only needed on sandbox builds.

We should not use typedefs in U-Boot. They cannot be used as forward
declarations which means that header files must include the full header to
access them.
Drop the typedef and rename the struct to remove the _s suffix which is
now not useful.
This requires quite a few header-file additions.

When no partition table is found, users should be warned so.
Warning that no device is available in this case could be misleading,
especially as it is the same error when no device is selected.

When we do not have CONFIG_BLK (or SPL/TPL) enabled there are very few
cases where we need the blk_legacy code linked in. To catch these, build
when we have CONFIG_HAVE_BLOCK_DEVICE set. In addition, we only need
cmd/blk_common.o to be linked in when we have CONFIG_HAVE_BLOCK_DEVICE
set, so make use of that directly.

When U-Boot started using SPDX tags we were among the early adopters and
there weren't a lot of other examples to borrow from. So we picked the
area of the file that usually had a full license text and replaced it
with an appropriate SPDX-License-Identifier: entry. Since then, the
Linux Kernel has adopted SPDX tags and they place it as the very first
line in a file (except where shebangs are used, then it's second line)
and with slightly different comment styles than us.
In part due to community overlap, in part due to better tag visibility
and in part for other minor reasons, switch over to that style.
This commit changes all instances where we have a single declared
license in the tag as both the before and after are identical in tag
contents. There's also a few places where I found we did not have a tag
and have introduced one.

config_fallbacks.h has some logic that sets HAVE_BLOCK_DEVICE
based on a list of enabled options. Moving HAVE_BLOCK_DEVICE to
Kconfig allows us to drastically shrink the logic in
config_fallbacks.h

There is already a macro LBAFU to aid formatted print with lbaint_t
variables. Let's use it in the common block command codes.

Most block devices provide a command (e.g. 'sata', 'scsi', 'ide') and
these commands generally do the same thing. This makes it harder to
maintain this code and keep it consistent.
We now have a block device interface which is either implemented by driver
model (when CONFIG_BLK is enabled) or with a legacy interface. Therefore
it is possible to handle most of what these commands do with generic code.
Add a new generic function to process block-device commands using the
interface type and the current device number for that type.
