An XHCI controller that sits behind an IOMMU needs to map and unmap
its memory buffers to do DMA. Implement this by inroducing new
xhci_dma_map() and xhci_dma_unmap() helper functions. The
xhci_dma_map() function replaces the existing xhci_virt_to_bus()
function in the sense that it returns the bus address in the case
of simple address translation in the absence of an IOMMU. The
xhci_bus_to_virt() function is eliminated by storing the CPU
address of the allocated scratchpad memory in struct xhci_ctrl.

Resetting an XHCI controller inside xhci_register undoes any register
setup performed by the platform driver. And at least on the Allwinner
H6, resetting the XHCI controller also resets the PHY, which prevents
the controller from working. That means the controller must be taken out
of reset before initializing the PHY, which must be done before calling
xhci_register.
The logic in the XHCI core was added to support the Raspberry Pi 4
(although this was not mentioned in the commit log!), which uses the
xhci-pci platform driver. Move the reset logic to the platform driver,
where it belongs, and where it cannot interfere with other platform
drivers.
This also fixes a failure to call reset_free if xhci_register failed.

So far we've been content with passing physical addresses when
configuring memory addresses into XHCI controllers, but not all
platforms have buses with transparent mappings. Specifically the
Raspberry Pi 4 might introduce an offset to memory accesses incoming
from its PCIe port.
Introduce xhci_virt_to_bus() and xhci_bus_to_virt() to cater with these
limitations, and make sure we don't break non DM users.

Some platforms, like MIPS Octeon, use mapped addresses (virtual address
!= physical address). On these platforms we need to make sure, that the
local virtual addresses are converted to physical (DMA) addresses for
the xHCI controller. This patch adds the missing virt_to_phys() calls,
so that the correct addresses are used.

xhci_writeq() makes the CPU->LE swapping only when addressing registers
in the xHCI controller address range and not in the local memory (RAM).
We need to use cpu_to_le64() here to ensure that the conversion is done
correctly.

While trying to use the U-Boot xHCI driver on the MIPS Octeon platform,
which is big endian, I noticed that the driver is missing a few endian

Some atypical users of xhci might need to manually reset their xHCI
controller before starting the HCD setup. Check if a reset controller
device is available to the PCI bus and trigger a reset.

In current code there is no cache flush after initializing the scratchpad
buffer array with the scratchpad buffer pointers. This leads to a failure
of the "slot enable" command on the rpi4 board (Broadcom STB PCIe
controller + VL805 USB hub) - the very first TRB transfer on the command
ring fails and there is a timeout while waiting for the command completion
event. After adding the missing cache flush everything seems to be working
as expected.

When U-Boot started using SPDX tags we were among the early adopters and
there weren't a lot of other examples to borrow from. So we picked the
area of the file that usually had a full license text and replaced it
with an appropriate SPDX-License-Identifier: entry. Since then, the
Linux Kernel has adopted SPDX tags and they place it as the very first
line in a file (except where shebangs are used, then it's second line)
and with slightly different comment styles than us.
In part due to community overlap, in part due to better tag visibility
and in part for other minor reasons, switch over to that style.
This commit changes all instances where we have a single declared
license in the tag as both the before and after are identical in tag
contents. There's also a few places where I found we did not have a tag
and have introduced one.

At present xHCI driver assumes LS/FS devices are attached directly
to a HS hub. If they are connected to a LS/FS hub, the driver will
fail to perform the USB enumeration process on such devices.
This is fixed by looking from the device itself all the way up to
the HS hub where the TT that serves the device is located.

So far LS/FS devices directly attached to xHC root port can be
successfully enumerated by xHCI driver, but if they are connected
behind a hub, the enumeration process fails to address the device.
It turns out xHCI driver still misses a part that in the device's
input slot context, all Transaction Translator (TT) related fields
are not programmed. The xHCI spec defines how to enable TT.
Now LS/FS devices like USB keyboard/mouse can be enumerated behind
a high speed hub.

xHCI spec says: the values of the 'route string' field shall be
initialized by the first 'Address Device' command issued to a
device slot, and shall not be modified by any other command.
So far U-Boot does not program this field, and it does not prevent
SS device directly attached to root port, or HS device behind an HS
hub, from working, due to the fact that 'route string' is used by
the xHC to target SS packets. But in order to enumerate devices
behind an SS hub, this field must be programmed.
With this commit and along with previous commits, now SS & HS devices
attached to a USB 3.0 hub can be enumerated by U-Boot.
As usual, this new feature is only available when DM is on.

signature to accept a pointer to 'struct usb_device', instead
of its members slot_id & speed, as the struct already contains

The scratchpad buffer array is used to define the locations of
statically allocated memory pages that are available for the
private use of the xHC. The xHCI spec explicitly mentions that
system software shall allocate the scratchpad buffers before
placing the xHC in to Run mode (Run/Stop (R/S) = ‘1’), however
U-Boot is missing this part.
This causes xHC on Intel platform does not respond the very first
'enable slot' command that is given to xHC and the 'enable slot'
command completion event TRB is never generated and xHC seems to
hang forever.

<asm-generic/...> is supposed to be included from <asm/...> when
arch-headers fall back into generic implementation. Generally, they

This adds the basic xHCI host controller driver with bare minimum
features:
- Control/Bulk transfer support has been added with required
infrastructure for necessary xHC data structures.
- Stream protocol hasn't been supported yet.
- No support for quirky devices has been added.
