internal: Don't explicitly warn against `semicolon_in_expressions_from_macros`
This has been warn-by-default for two years now and has already been added to the future-incompat lints in 1.68.

Streamline MIR dataflow cursors
`rustc_mir_dataflow` has two kinds of results (`Results` and `ResultsCloned`) and three kinds of results cursor (`ResultsCursor`, `ResultsClonedCursor`, `ResultsRefCursor`). I found this quite confusing.
This PR removes `ResultsCloned`, `ResultsClonedCursor`, and `ResultsRefCursor`, leaving just `Results` and `ResultsCursor`. This makes the relevant code shorter and easier to read, and there is no performance penalty.

TokenMap -> SpanMap rewrite
Opening early so I can have an overview over the full diff more easily, still very unfinished and lots of work to be done.
The gist of what this PR does is move away from assigning IDs to tokens in arguments and expansions and instead gives the subtrees the text ranges they are sourced from (made relative to some item for incrementality). This means we now only have a single map per expension, opposed to map for expansion and arguments.
A few of the things that are not done yet (in arbitrary order):
- [x] generally clean up the current mess
- [x] proc-macros, have been completely ignored so far
- [x] syntax fixups, has been commented out for the time being needs to be rewritten on top of some marker SyntaxContextId
- [x] macro invocation syntax contexts are not properly passed around yet, so $crate hygiene does not work in all cases (but most)
- [x] builtin macros do not set spans properly, $crate basically does not work with them rn (which we use)
~~- [ ] remove all uses of dummy spans (or if that does not work, change the dummy entries for dummy spans so that tests will not silently pass due to havin a file id for the dummy file)~~
- [x] de-queryfy `macro_expand`, the sole caller of it is `parse_macro_expansion`, and both of these are lru-cached with the same limit so having it be a query is pointless

- [x] fix eager macro spans and other stuff
- [x] simplify include! handling
- [x] Figure out how to undo the sudden `()` expression wrapping in expansions / alternatively prioritize getting invisible delimiters working again

Exhaustiveness: allocate memory better
Exhaustiveness is a recursive algorithm that allocates a bunch of slices at every step. Let's see if I can improve performance by improving allocations.
Already just using `Vec::with_capacity` is showing impressive improvements on my local measurements.

move exposed-provenance APIs into separate feature gate
We have already stated explicitly for all the 'exposed' functions that

However, they were part of the same feature gate and still described as part of the strict provenance experiment. Unfortunately, their semantics are much less clear and certainly nowhere near stabilization, so in preparation for an attempt to stabilize the strict provenance APIs, I suggest we split the things related to "exposed" into their own feature gate. I also used this opportunity to better explain how Exposed Provenance fits into the larger plan here: this is *one possible candidate* for `as` semantics, but we don't know if it is actually viable, so we can't really promise that it is equivalent to `as`. If it works out we probably want to make `as` equivalent to the 'exposed' APIs; if it doesn't, we will remove them again and try to find some other semantics for `as`.
Add substring API for `OsStr`
This adds a method for taking a substring of an `OsStr`, which in combination with [`OsStr::as_encoded_bytes()`](https://doc.rust-lang.org/std/ffi/struct.OsStr.html#method.as_encoded_bytes) makes it possible to implement most string operations in safe code.

Handle recursion limit for subtype and well-formed predicates
Adds a recursion limit check for subtype predicates and well-formed predicates.
`-Ztrait-solver=next` currently panics with unimplemented for these cases.
These cases are arguably bugs in the occurs check but:
- I could not find a simple way to fix the occurs check
- There should still be a recursion limit check to prevent hangs anyway.
