Add Trusted Origins to CSRF Handler ( #2301 )

Enables the user to add addresses that they expect incoming connections
to come from. Helpful in cases where TLS termination is handled by a
load balancer upstream, rather than the application itself.

Added a contact_address entry in config.json to support transparency efforts (ref #1057 ).

Also added a warning in the case where a contact address isn't provided, and fixed the JSON formatting of the configuration.
