Move accessibility setup to accessibility module (#12784)

# Objective

- Reduce the size of `create_windows` and isolate accessibility setup
logic.

## Solution

- Move accessibility setup for new windows to the `accessibility`
module.

## Comments

This is a small refactor, no behavior changes.
